Dr. Abdurrahman ÖZGÜR,a Dr. Levent N. TÜRKERİa
a Üroloji AD, Marmara Üniversitesi Tıp Fakültesi, İSTANBUL Benign prostatic hyperplasia (BPH) is present in about 70% of men over the age of 60. Symptomatic BPH is thought to be mainly due to the bladder outlet obstruction. The results of long term follow up studies and placebo controlled clinical trials indicated that BPH is a progressive process for many men. In addition, men with lower urinary tract symptoms (LUTS) without any treatment, the risk of acute urinary retention is about 1-1.5% per year.
Treatment of BPH is performed mainly two groups of drugs with different mechanisms of activity. These are alpha-1 receptor blockers and 5-alpha reductase inhibitors. There are not too many multi center, double blind, long term, placebo controlled studies about the use of these two drugs as a combination therapy. The results of these studies revealed a high tolerance rates for these drugs, even in combination therapy for a long period.
Medical therapy is the first-line treatment in patients with lower urinary tract symptoms. However, patient characteristics are the most important factors in decision making for teh drug to use. Long term combination therapy may be a viable option, especially for those patients who pose a high risk for future surgical intervention. Keywords: BPH, alpha receptor blocker, 5-alpha reductase inhibitorTurkiye Klinikleri J Surg Med Sci 2005, 1(1):71-74
