Current Management Of Metastatic Nonseminomatous Testicular Cancer

aÜroloji AD, Çukurova Üniversitesi Tıp Fakültesi, ADANA Nonseminomatous germ cell tumors (NSGCT) of testis are the most common neoplasm in male patients younger than 40 years. The overall incidence was doubled during the past 40 years. Although the incidence of testicular NSGCT increased, mortality rates have dramatically fallen aproximately by %70 since the advent of the cisplatin based chemothrapy regimens in the 1970’s. The surgical removal of residual disease after cisplatin based chemotherapy continues successfully.
The testicular NSGCT is frequently metastasised to the retroperitoneum and subsequently to the mediastinal lymp nodes through the lymphatics. Hematogenous metastases may occur to the lung, bone or brain. Supradiaphragmatic metastasis may be seen before and during the chemotherapy in the 10 – 20%of the patients with NSGCT of testis. Surgical removal of the mediastinal and pulmonary masses that are radiographically diagnosed may increase the rate of long-term survival.Keywords: Testicular cancer, chemotherapy, residual tumor resectionTurkiye Klinikleri J Surg Med Sci 2005, 1(9):77-83
